Passport Application Checklist

To Obtain a New Passport, You Will Need the Following Items:

1. Completed Form DS-11, Application for a U.S. Passport
      • Form DS-11: Application for a U.S. Passport Print a blank form or Complete online and print
      • All other forms you might need may be found at the U.S. Department of State website
      • Forms are also available at the Adult Public Services Desk or online at travel.state.gov
      • Forms must be completed in black ink or filled in online and printed out prior to the appointment.
      • DO NOT SIGN THE FORM until requested to do so by an authorized agent.

2. Evidence of U.S. Citizenship (one of the following):
      • Previously issued, undamaged U.S. Passport (If renewing a passport for a minor, bring both the previous passport and copy of birth certificate.)
      • Certified birth certificate issued by the city, county, or state
      • Consular Report of Birth Abroad or Certification of Birth
      • Naturalization Certificate
      • Certificate of Citizenship

      Birth Certificate requirements (Copies are not acceptable)
         • Full name of the applicant
         • Date of birth and Place of birth
         • Raised, embossed, impressed, or multicolored seal of the issuing authority
         • Full names of the applicant’s parents
         • Registrar’s signature
         • Date certificate was filed with the registrar’s office (must be within one year of birthdate)

3. One Color Passport Photo
      • Applicants can take photos during appointments or obtain photos at a local drugstore or wholesale club. Applicants under 3 should obtain photos prior to their appointments.
      • Picture must be 2 x 2 inches in size, taken within the past 6 months, and show current appearance, full face, with a plain white or off-white background, taken in normal street attire. NO GLASSES in photo.

4. Identification (one of the following):
      • Valid Driver’s License
      • Previously issued, undamaged U.S. Passport
      • Current Military ID (military and dependents)
      • Current city, state or federal Government ID (bring a secondary source of ID such as a social security card)

5. Applicable Passport Fees:
      • Separate payments must be made for the Passport Fee and the Execution Fee
      • Passport Fee payable to “U.S. Department of State” must be a Personal check, money order, or bank draft. (Note: A separate check must be attached to each application)
      • Execution Fee payable to “Twinsburg Public Library” can be Personal check, money orders, bank drafts, exact cash or credit cards
      • Check the Passport Fees chart at the U.S. Department of State website for current fees.

Additional Information for Minors under 18
      • All children under age 16 must apply for a passport in person with two parents/guardians.
      • Parents/guardians must present proper Identification, such as Driver’s licenses or passports.
      • You must also submit documentation that shows parental relationship by listing the parent(s) or legal guardian(s) of the child applying. The following may be used to show parental relationship: U.S. or foreign birth certificate, Consular Report of Birth Abroad or Certification of Birth, Adoption decree, Divorce/Custody decree. Please note: Some documents, like a U.S. birth certificate, show both U.S. citizenship and parental relationship. These documents must be originals or certified copies (not photocopies).
      • A parent/guardian who cannot attend an appointment with the child may give permission for execution by submitting a signed and notarized copy of Form DS-3053 “Statement of Consent", as well as a photocopy of the front and back side of their ID.
      • Minors aged 16 or 17 only need one parent/guardian to be present at their appointments.
